Job Description:

Lebanon Center Rehabilitation & Healing is seeking a dedicated and detail-oriented MDS Coordinator, RN to join our team. In your role, you will be integral to our care coordination, focusing on patient assessments and ensuring accurate documentation in compliance with regulatory frameworks. Your work will directly contribute to the optimized care plans that uphold our mission of providing excellent recovery and supportive environments. As the MDS Coordinator, you will collaborate across teams to enhance patient outcomes and center operations. Conduct and coordinate the development, completion, and transmission of resident assessments (MDS) in accordance with current federal and state regulations. Identify resident needs and collaborate with other healthcare team members to design comprehensive, individualized care plans. Maintain accurate and timely patient records and documentation as per regulatory requirements. Participate in quality assurance teams to review care outcomes and process improvements. Train and assist staff nurses and other team members on changes in assessment and care planning procedures. Proficient in MDS 3.0 Strong analytical and problem-solving skills Excellent verbal and written communication Effective time management and organizational skills Deep understanding of federal and state healthcare regulations Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license Certification as a Resident Assessment Coordinator (RAC-CT) preferred
